The following is a list of all forms used to enter the Alaska Science & Engineering Fair.
Note: Official International Science and Engineering Fair (ISEF) forms are also available on the ISEF site: https://student.societyforscience.org/forms.

Forms for Elementary School (Grades K – 5, grade 6 if attending elementary school):

- Abstract Contest Entry Form Note: Students in grades 4-6 are gently encouraged to write an abstract for their project portfolio. They are invited to enter a second copy in the abstract contest.

Every senior high student needs the event forms:
Every senior high student needs forms 1, 1A, 1B, and the abstract form
- Checklist for Adult Sponsor (1)
- Student Checklist/Research Plan (1A)
- Approval form (1B)
- ISEF Abstract and Certification form – Note: Students in grades 9-12 are required to write an abstract for their project portfolio. Note: This form will be also be used for the special category Alaska Professional Communicators abstract contest for senior high projects. To enter the contest, please staple a local abstract contest entry form to a second copy of your abstract (on the ISEF Abstract and Certification form) and turn it in at registration. Please keep the first copy of your abstract with your project portfolio. If you win the top prize at the Alaska Science & Engineering Fair, you will be asked to enter your abstract text using ISEF’s on-line version of this form.
Some senior high projects need forms 1C, 2, 3, 4, 5A/5B, 6A, 6B, and 7. Use the The Intel ISEF Rules Wizard to help determine which forms apply to your senior high project.
- Regulated Research Institutional/Industrial Setting Form (1C)

- Human Participants form (4) and Human Informed Consent Form If informed consent from internet sources is needed, use both parts of this form.
